Incident reporting
Inizio employees and contingent
workers are required to report and
investigate all work-related health
and safety incidents, including near
misses. Our businesses escalate
incidents as appropriate to senior
management and the Group.
Our business units report incidents
monthly via our Group reporting
database, which we extended across
the whole organization in 2023. We
monitor trends related to incident
performance and use this information
to identify areas for improvement.
Across Inizio, 66 incidents were
reported in 2023, of which 64 were
accidents, i.e., cases with injury:
Most injuries were classified as minor;
• There were 0 high-consequence
work-related injuries (fatalities or
injuries with longer recovery times
of 6 months or more).
• 21 recordable work-related
incidents occurred, i.e., injuries
resulting in lost work days, for
which the corresponding incident
rate was 0.18, per the calculation.

The incident causes and injury types reported during the past year are
indicated in the charts below:
Due to our business merger and
restructuring, comparable incident
data is not available for 2022 versus
2023; however, annual data comparisons
will be reinstated from 2024.
The top root causes of incident, as
shown, accounting for c.44% of all
incidents reported in 2023, were slips,
trips, and falls along with road traffic
accidents while driving for work.
The nature of injury chart across shows
that the majority of injuries sustained
were relatively minor in nature, e.g.,
cuts, bruises, sprains, orstrains.
The more prevalent inherent risks to
health and safety associated with
our business services include driving
for work, incidents within clinical
support environments and/or while
lone working. We have controls in
place to either eliminate or mitigate
risks as appropriate and we continue
to make improvements, for example,
through our enhanced driver-safety
programs and other health and
safety management processes.
Driver safety
We revised our Driving for Work Policy
during 2023 and extended it across the
entire organization. We require our
businesses to include driving for work
in health and safety risk assessments
and to consider the use of both
company fleet cars and personal
vehicles for work (the latter is known
as“grey fleet”).
Inizio Engage vehicle fleet
The majority of the Inizio Engage
workforce is field based, requiring
usto operate vehicle fleets in 11
countries. More than 1,000 company
vehicles were driven across 2023
covering greater than 22 million
miles. Within Inizio Engage, many
drivers use their own vehicles for
work-related transport.
Driving is the largest cause of incidents
at Inizio Engage, reflecting the high
number of employees who drive for
work and the amount of business miles
driven. Driver safety management,
therefore, is a business priority to
protect our employees and others.
Incident root cause
Nature of injury
Inizio Engage drivers are required
totake part in online driver safety
management programs, which
include risk assessments, training,
and coaching. The program was
extended to new countries and
modules were further rolled out
across existing countries during
2023. All 11 countries where we
operate vehicle fleets are covered
and we have plans to expand the
scope of the training and reviews,
tofurther improve safety standards.
Inizio Engage is a member of the
Network of Employers for Traffic
Safety (NETS). We measure the
number of collisions per million
miles (cpmm) and the injuries from
collisions per million miles (ipmm),
and benchmark our performance
against other organizations operating
car fleets. Based on NETS’ benchmark
data, our cpmm figure improved in
2022 vs 2021.
The Global Director of EHS for
Inizio Engage chairs a quarterly
fleet managers’ meeting to look
at safetyperformance data and
sharebest practice.
